  • Patient Dashboard --> Print Patient Info --> Add PMH Checkbox

• • Patient Dashboard --> New Fax --> Add patient Information Checkbox  --> Add PMH Checkbox

• • Patient Dashboard --> Patient Basics --> Past Medical History (PMH)

• • Patient Dashboard --> Medical Record --> Problems and Procedures --> Past Medical History and Previous Diagnoses Grids

    • Right click image below and select "Open image in New Tab" to see detail. 

• Nurse&Support and Provider Modules

  • Appointment Patient Summary contains "Past Medical History" and "Previous Diagnoses" grids. Both grids can be selected to auto-collapse by marking checkbox.

• •  History & Symptoms Tab

    • Contains "Past Medical History" and "Previous Diagnoses" grids.
    • Grids function the same as described in Patient Dashboard --> Medical Record --> Problems and Procedures --> Past Medical History and Previous Diagnoses Grids
    • Additional button "Add to Visit" allow for current problems to be made visit pertinent.
